Job DescriptionEasterseals Blake Foundation is seeking direct professionals to work in a collaborative environment supporting individuals with disabilities in their homes and the community.We offer competitive wages, great benefits including but not limited to Sick Time, PTO, Health, Dental and Retirement.Job Summary: Supported Living Direct Professionals work directly with people in their homes and in the community, assisting them to develop a sense of identity, self-worth and independence.ESSENTIAL DUTIES AND RESPONSIBILITIES:Assistance in the following areasMeal prepUpkeep of home, through innovation, take daily chores/duties and turn them into interesting individual learning experiences.Community activities, Plan and conduct incredible activities and everyday fun.Grocery ShoppingBill payAppointmentsTransportationSocial gatheringsMedication managementPersonal hygieneFunction as an enthusiastic, competent, contributing member of EBF staff.Attend and participate in required staff meetingsPromote and follow practices that support health and safety of staffDevelop and maintain positive relationships with individuals we serve, team members, families and state agencies. Encourage positive team work and problem solve in a non-hostile mannerComplete paperwork within required timelinesAttend all necessary training within the time lines established at the time you were hiredWrite in /\"in-house/\" communication log after every shift worked. Maintain and monitor daily communication log between day program and Supported Living programMaintain agreed upon work schedule, be punctual, and use time effectivelyUse supervision effectively, accept feedback in a professional manner, and accept responsibility for job performanceCommunicate essential information with ESBF team leads and supervisors accurately and in a timely mannerUnderstand and maintain private and sensitive information within the confines of need-to-know parameters.KNOWLEDGE, SKILLS AND ABILITIESAbility to work flexible hours including early mornings afternoon and evening hours and at least 1 weekend day;Ability to speak and write effectively in English.Sensitivity to cultural diversity and differencesAbility to complete job responsibilities autonomously without daily oversightAn ability to acknowledge, respect, and build upon the diversity of person served and their families with regard to such factors as race, ethnicity, family constellation, gender, socioeconomic status, values, and traditions.An ability to teach others using anticipatory guidance, modeling, coaching, reflection, and role-play EXPERIENCE / EDUCATION / LICENSE REQUIREMENTSAt a minimum a high school diploma or equivalentProvide and maintain Valid Arizona Driver s License, Registration & InsuranceAbility to maintain a valid Arizona Fingerprint Clearance Card at all timesExperience providing personal care and/or working with individuals with a developmental disability
